TITLE: Medical Records Technician

JOB OVERVIEW: Responsible for ensuring completion of the medical record through review of specific accounts and assignment of deficiencies as identified. Responsible for reporting deficiencies to physicians and working with them to complete those records. Responsible for evaluating potential duplicate medical records and merging as necessary. Responsible for assisting outside review agencies with record pulls as needed. Responsible for training new staff or assisting staff and physicians with access to ChartMaxx as needed.

AREA OF ASSIGNMENT: Health Information Management (Medical Records)

HOURS OF WORK: Monday - Friday days, as assigned.

RESPONSIBLE TO: Manager, Health Information Management


PREREQUISITES:

  1. High School Graduation or equivalent.

  2. Demonstrates skill and typing and knowledge of PCs.

  3. Excellent customer service skills.

  4. Demonstrated ability to attend to detail and perform accurate work.

  5. Demonstrated ability to effectively communicate in writing and verbally in the English language. Ability to spell accurately and write legibly.

QUALIFICATIONS:

  1. Previous experience in a Medical Records setting or medical office preferred.

  2. Medical terminology knowledge preferred.

  3. Neat and well-groomed appearance.

  4. Demonstrated ability to maintain records accurately and keep all records confidential.

  5. Demonstrated ability to interact professionally on the phone and in person with staff, doctors and supporting departments.

  6. Demonstrated ability to learn tasks and handle responsibility.

  7. Able to carry out assignments and exercise good independent judgment.

  8. Demonstrated ability to decipher handwritten notes.

  9. Excellent organizational and time management skills.

  10. Ability to use McKesson Star, Epic, Excel, Word, and/or ChartMaxx preferred.

  11. Regular and punctual attendance is a condition of employment.

  12. Demonstrated ability to follow Service Culture Guidelines.

UNIQUE PHYSICAL/MENTAL DEMANDS, ENVIRONMENT, AND WORKING CONDITIONS:

Physical requirements may include moderate lifting of files. Must be able to bend, stoop, lift, reach, push and pull. Involves sitting at a keyboard at least 8 hours per day.

PERFORMANCE RESPONSIBILITIES:

  1. Reviews records for signatures and other pertinent information, indicating deficiencies on ChartMaxx.

  2. Carefully evaluates duplicate medical records and merges them if required.

  3. Pulls records for studies, maintains log of requests, communicates with requester and arranges conference room or access to a computer, as needed

  4. Assists physicians when necessary with accessing of records on ChartMaxx.

  5. Sends weekly reminder notices to physicians with potentially delinquent medical records.

  6. Communicates with physician offices when records are incomplete.

  7. Provides training to new clerical staff members when requested.

  8. Maintains confidentiality of all accessible patient financial or medical records information.

  9. Assures that regulatory compliance monitoring tool is completed and submitted monthly to manager.

  10. Demonstrates the awareness of the importance of cost containment for the department. Provides suggestions regarding process or quality improvement opportunities to department manager.

  11. Performs all job functions in a manner consistent with Valley's cultural expectations defined by the Service Culture Guidelines.

  12. Other duties as assigned to facilitate accurate, timely patient account management
